Encore Healthcare was founded in January 2015 by a team of Respiratory Therapist, Pulmonologists, and healthcare business experts with extensive expertise in post-acute care. Led by Zach Gantt, RRT, CEO, Keisha Harper, COO & Dan Easley, Chief Strategy Officer. Encore was founded with the concept of taking a unique Pulmonary Population Health model and developing technology around it to drive efficiencies and improved patient outcomes. Having been successful as providers for the better part of the last decade, Encore's principles decided to take the model to the top respiratory companies in the US, creating a national network of outcome based respiratory services providers. Encore's principles have published 45+ abstracts in every notable peer review journal in the US and abroad creating a new standard of care for COPD management. Using technologies like vitality focused telemonitoring, telemedicine and Encore's proprietary software, our mission is to deploy Respiratory Therapists 24/7 in every setting both live and virtually. Creating the next generation Respiratory Care delivery models.

Encore national network of outcome based Respiratory Providers allows for the deployment of our programs with consistent outcomes anywhere in the US. Encore's network allows us to work with local, regional, & even national health plans, Integrated Delivery Networks & Skilled Nursing Facilities (SNF) to implement solutions using our Nexus software platform.

Encore’s Nexus platform is a web-based respiratory patient mgmt software, activity sensing, symptom monitoring technologies that incorporate risk stratified & proven respiratory protocols. Our network of respiratory providers is essentially an extension of Encore's programs & Services. The network is recruited, trained, & certified by Encore, creating consistency in clinical expertise, protocols & outcomes. This consistency combined with the 300+ clinicians across the US allows for a scalable model w/ rapid ROI for all stakeholders and more importantly better outcome for our patients.

    • Transitional Care program allows for a low cost extension of a hospitals clinicians through automation, allowing access to patients within 30-90 days post discharge w/o the high cost of sending clinicians into the home.
